Reporting Documents for Colleges and Universities

Participating colleges and universities are asked to use the documents listed below to report on the use of funds for their Daniels Opportunity Scholarship grant at the end of each school year. Please fill out each of the reports and either fax or e-mail the documents to the attention of Tami Brown at the address listed at the bottom of the document.
