Application forms and guides for Charter Schools
Charter School Grant applicants must fill out the Grant Application Form (proposal checklist and organization summary form), Charter School Outcomes Template and School Performance Table, the school’s Charter Application Executive Summary with Evidence of Authorization, School Organization Chart, and Business Plan (5 Year Preferred).
